Jefferson House embodies the whole-person approach, offering comfort, security and individualized healthcare to avoid long hospital stays through nursing, intensive rehabilitative therapy, nutritional counseling, self-maintenance and psychological support necessary for re-establishing independence in daily living.
Nursing Services
The nursing staff of Jefferson House is committed to providing high quality care. The nurses, as members of the interdisciplinary team, work collaboratively in the development of a care plan for each patient. This care plan directs the team to assist the patient in achieving the greatest state of wellness possible. The nurses also serve as a patient advocate and resource to the patient, family and interdisciplinary team.
Social Services
Social workers are available to provide supportive counseling to patients and families, in both an individual and group setting. Our social service staff works closely with the interdisciplinary team in meeting our patient's emotional and social needs. Social workers also work with patients, families, physicians and community programs in order to establish appropriate discharge plans for rehabilitative patients.
Spiritual Services
Another important component of our total care concept is meeting the spiritual needs of the patients. Clergy from area churches and synagogues ensure that their patients may participate in the practice of their faith.
Therapeutic Recreation
Recreation is a vital part of our patient's total care plan. The recreation therapists work with all of the professional staff to determine the most appropriate therapeutic recreation program for each patient. Our objective is to provide them with activities they truly enjoy while satisfying therapeutic goals.
